the preventive care and routine wellness benefits described below are
covered at 100%, and are not subject to a copayment or coinsurance.

• Annual check-up for members age 7 and older
• Annual gynecological exam and Pap test
• Annual mammogram for members age 35 and
over
• Annual colorectal cancer screening
• Annual prostate exam and Prostate Specific
Antigen (PSA) test for men age 40 and over
• Supplemental Accident Coverage (as defined
in the policy) covering 100% of the first $750 of allowable charges per
covered person for emergency outpatient treatment of traumatic bodily
injuries resulting from an accident
